A. The U.S. National Cemetery Interment Control Form identifies his assigned unit as the 248th Coast Artillery, Fort Worden, Harbor Defenses of Puget Sound.

B. Hospital Admission Records indicate that TEC5 Calver was admitted to medical facilities at Ft. Worden, died on December 13, 1942, likely but not confirmed, as a result of of complications of appendectomy.
