A. His OQMC Form 623, Application For Headstone or Marker, identifies his assigned unit as Headquarters Company, 1st Battalion, 123rd Infantry Regiment, 33rd Infantry Division and date of death as April 1, 1945.
B. At the time TEC5 McConnell was Killed In Action the 33rd Infantry Division was conducting combat operations on Luzon, Philippine Islands from February to May 1945.
C. Hospital Admission Records indicate that TEC5 McConnell was "Not in Medical Installation Prior to Death" and post mortem examination found a "puncture or stab wound".
