A. His OQMC Form 623, Application For Headstone or Marker, identifies his assigned unit as Company D, the 355th Infantry Regiment, 89th Infantry Division and date of death as March 11, 1945.
B. On March 11, 1945 the 89th Infantry Division was in Luxembourg moving up to the Sauer River preparing to cross the border into Germany. Note that his Find A Grave listing states
that his death occurred in Germany.
C. Hospital Admission Records record that PFC Frazier was "Not in Medical Installation Prior to Death" and post mortem examination found wounds caused by a "Land Mine".
